At Phillips County Hospital, we understand that some patients need skilled care but may have difficulty traveling to appointments. While we are not a full home health agency, we offer Visiting Nurse Services to support eligible patients in the comfort of their own home.
Who We Serve
Our visiting nurse provides care to patients who are:
- Homebound or have difficulty leaving home safely
- Recently discharged from the hospital or clinic
- In need of short-term skilled nursing services
- Under the care of a provider with a valid order for services
Services May Include
Our skilled nurse can provide a variety of medically necessary services, including:
- Wound care and dressing changes
- Medication management and education
- Post-hospitalization follow-up care
- Monitoring of chronic conditions (such as diabetes, COPD, or heart conditions)
- Vital signs assessment and health status monitoring
- Patient and caregiver education
- Coordination with your provider and care team
How It Works
- A provider referral/order is required
- Our team reviews the request to ensure services are appropriate and safe to provide in the home
- A nurse will contact you to schedule visits
- Visits are intermittent and based on medical need, not continuous care
Important Information
- This service is designed for short-term, skilled care needs
- We do not provide 24/7 care or custodial services
- Availability may vary based on staffing and patient needs
